ABSTRAK Kualitas produk merupakan salah satu hal yang penting guna mempertahankan strategi bisnis di era globalisasi saat ini. Kapontren Nurul Iman merupakan salah satu koperasi pondok pesantren yang bergerak dibidang air minum dalam kemasan (AMDK). Kapontren Nurul Iman sendiri masih memiliki masalah dalam menciptakan kualitas bagus pada produk. Permasalahan tersebut dapat dilihat pada air minum dalam kemasan merek “AIMAN” dengan kemasan 220 ml yang masih banyak kegagalan yang terjadi selama proses produksi berlangsung. Pada penelitian ini diharapkan dapat membantu perusahaan dalam mengurangi jumlah kegagalan yang terjadi dengan menggunakan metode DMAI dimana tahapnya yaitu Define, Measure, Analize, Improve (DMAI). Serta menggunakan salah satu tools Failure Mode Effect Analize (FMEA) yang dapat memberikan usulan kepada kapontren Nurul Iman untuk mengurangi timbulnya kecacatan berulang kembali berdasarkan data yang diperoleh dari bulan Januari 2015 – Maret 2015. Berdasarkan hasil dari penelitian menggunakan metode DMAI dan FMEA terdapat tiga jenis cacat yang memiliki jumlah cacat terbesar yaitu cacat cuo, cacat oenutup cup, cacat ketidaksesuaian volume dari ketiga jenis cacat tersebut nilai cacat tertinggi terletak pada cacat bentuk cup kurang sempurna. Rekomendasi perbaikan pada jumlah cacat terbesar terletak cacat ketidaksesuain volume pada proses holder yaitu memeriksa holder sebelum dan setiap melakukan proses produksi guna mencegah timbulnya kecacatan yang disebabkan oleh holder. ABSTRACT The product quality is one thing that is important to maintain a business strategy in the current era globilisasi. Kapontren Nurul Iman is one of the boarding school cooperatives engaged in bottled drinking water (bottled water). Kapontren Nurul Iman itself still has the problem of creating a good quality product. Such problems can be seen in the bottled water brand "Aiman" by packing 220 ml which is still a lot of failures that occurred during the production process. In this research is expected to help the company in reducing the incidence of failures by using DMAI where stages are Define, Measure, Analyze, Improve (DMAI). As well as using one of the tools Failure Mode Effect Analyze (FMEA) to provide recommendations to kapontren Nurul Iman to reduce the incidence of recurrent disability is based on data obtained from January 2015 - March 2015. Based on the results of research using FMEA DMAI and obtained the number of defects the key is defective or CTQ cup of 7548 with the number of defects, defect cover cup with the number of defects by 6826, defective mismatch cup volume with the number of defects by 4499, of the three types of defects are defects highest value lies in the deformed cup less than perfect. Recommendations for improvements in the number of nonconforming defect biggest flaw lies in the holder that volume check holder before and every production process in order to prevent disability caused by the holder.
